Dr. John C. Carrozzella

Dr. Carrozzella was born in Philadelphia in 1956 and grew up in Wallingford CT.  There, he attended The Choate School in preparation for college.  He graduated from Yale University in 1978 with Honors in Molecular Bio-Physics and Biochemistry.  In 1982 Dr.  Carrozzella earned his MD from the University Of Cincinnati College Of Medicine.  At UC, he placed in the top 5% of his class and he achieved a distinguished status by earning the Alpha Omega Alpha Honor Society award in his junior year, one of only 10 students in a class of 200 to do so.  He was selected as the number one choice by the University of Cincinnati Residency in Orthopaedic Surgery, a significant accomplishment in a very competitive subspecialty field.  By placing in the top 5% of his Board Certification group, Dr.  Carrozzella again showed a superior medical aptitude.  In the final phase of his early training, he was selected as the very first fellow of what has become one of the premier Hand Surgery Training programs in United States, the Mary S Stern Fellowship in Hand and Micro-Surgery.

Dr.  Carrozzella was a practicing Orthopaedic Surgeon for more 25 years. Since 2000, he has been located in the Tampa Bay-Orlando corridor.  For the last several years, he has focused on interests in nontraditional medical specialties such as Hormone Replacement, Sexual Health for Men, Sexual Health for Women, and Dysfunction, Non-surgical Cosmetics, Health and Wellness, Age Management and Integrative Medicine.  Over the years he has been a member of a number of medical associations which have included: the American Academy of Orthopaedic Surgeons, American Board of Orthopaedic Surgery, the American Society for Surgery of the Hand, and the American College of Surgeons.  In 2014 Dr.  Carrozzella became an Advanced Fellow of the American Academy of Anti-Aging and Aesthetic Medicine, a member of the International Society for Sexual Medicine and the International Society for the Study of Women’s Sexual Health.

In order to re-direct his career and to achieve the necessary expertise in his current fields of interest, Dr. Carrozzella completed several hundred hours of continuing education credits; In August 2016, Dr. Carrozzella was awarded a Master’s Degree in Metabolic and Nutritional Medicine from the University of South Florida; in 2013 he became a diplomat of the American Board of Anti-Aging and Regenerative Medicine and he carries a certificate in Sexual Medicine from the American Academy of Anti-aging Medicine and a Certificate in Advanced Endocrinology from the University of South Florida.  He is an expert in Platelet rich Plasma Procedures and is also a certified practitioner of the Vampire Face Lift, Breast Lift, Facial, O-shot and Priapus shot.

Nurse Practitioner Erin Tongue, FNP-C

Erin Tongue is a Family Nurse Practitioner and the newest provider to join The Florida Center for Hormones and Wellness team. After witnessing her husband’s incredible transformation with hormone replacement two years ago, Erin knew this was the only kind of medicine she wanted to practice as a provider.

 

Her first bachelor’s degree, Speech Communications, was completed in May of 2000 from SUNY Potsdam in Potsdam, NY.  In May of 2018, Erin graduated from the BSN Program at the Decker College of Nursing and Health Sciences at Binghamton University, and her MSN, FNP degree was recently completed in August, 2020 from Herzing University.

Nurse Practitioner Suzette Boyette, APRN

Suzette Boyette, APRN board certified Women’s Health Nurse Practitioner, is the newest member of our FCHW family. Throughout her entire nursing career of over 30 years, she has always dreamed of working in a place where patients’ total wellness is a priority which led her to our clinic.

 

Suzette is a Florida Gator through and through. She graduated with her bachelor’s in nursing degree 1993 and her master’s in nursing 1996 both from the University of Florida. She has worked in a variety of settings from private Ob/Gyn practices to a reproductive endocrinology practice to Student Health at the University of Florida. Caring for patients with a holistic team approach to balance body, mind, and spirit throughout the various seasons of life has always been her philosophy. Suzette’s passion is being an advocate for her patients so they can learn how to live in joy each day with the tools necessary to make the best decisions for their health and well-being.

Suzette is a member of the American Academy of Nurse Practitioners, the Nurse Practitioner Women’s Health organization, and the Florida Nurse Practitioner Network. Other hats Suzette has worn in the past include being adjunct faculty at UCF College of Nursing, a magazine columnist for a local parenting magazine as well as a podcaster with her husband. Lindsey Cieslak, APRN, earned her Bachelor’s degree in Nursing from the University of Central Florida, which started her career in nursing by working in the Neonatal Intensive Care Unit for over 4 years. She then moved on to receive her Master’s in Family Nurse Practitioner from Jacksonville University. She began her career at Foundations at Maitland specializing in aesthetic treatments and hormone replacement therapy. Currently, she is certified in injectables (Botox and Dermal filler), Inmode Morpheus (Face/body/vaginal), Lumecca, DIVA vaginal laser, Halo laser, Hydrafacial, PRP, Laser hair reduction, Sofwave and Body by BTL.

Johannah Katz is a Registered Dietitian/Licensed Dietitian Nutritionist, and certified 3×4 Nutrigenomic practitioner with a master’s degree in applied nutrition and dietetics from Union Institute and University. Johannah is trained with a certificate in Integrative and Functional Medical Nutrition Therapy and is the author of the children’s book “Lana’s Dinner”, which encourages young readers to “eat the rainbow”.

A graduate of the Boston Conservatory, Johannah is a second career dietitian with a professional background in musical theatre and vocal performance.
